Welcome to Romulus! It is our goal to make this transition as quick and positive as possible. Please take a moment to gather the following items: - Proof of child’s age (birth certificate, passport, medical card, certificate of baptism, or other acceptable proof of age)
- Proof of residency (Drivers license, lease/deed, rental contact, utility bill (gas, electric, etc.), letter from landlord,Mortgage or closing statement, Pay Stub with current address, Income tax form, Military Orders, DSS Declaration, Documents issued by federal, state or local agencies, Voter registration documents) A parents name must be on the proof of residency!
- Proof of custody or guardianship (if applicable)
- Immunization records & Last Physical (Must have taken place within the past 12 months)
You will need electronic versions of these documents so you can attach them to the registration. If not already in electronic form, you can scan or take a picture of these items. All applications are considered incomplete until all documentation listed has been provided.
